Weight Loss Clinic Modderfontein: Doctor, Dietitian and Physiotherapist Under One Roof
Equilibrium Health is a doctor-led weight loss clinic in Modderfontein, Johannesburg. Your programme starts with a one-hour consultation and InBody body composition analysis, continues with dietitian-integrated nutrition care and follow-up visits about every four weeks, and includes on-site blood tests through LANCET Laboratories, next door to the clinic.
You choose between the Equilibrium Diet (plant-based) and the Reset Diet (anti-inflammatory), and the plan is reviewed at each visit using your measurements and body composition results. Our dietitian, Charné, is available for consulations and to guide you through your weight loss journey. We submit medical aid claims directly to your scheme.
Dr Nick Petropoulos, Obesity Medicine Physician.
Our in-house lead physician manages the full medical and pharmacotherapy side of your care, from initial metabolic assessment through to ongoing prescribing decisions.
InBody body composition analysis.
In-house testing equipment measures fat mass, muscle mass, and visceral fat, tracked over time, not just a number on a scale at each visit.
Dietitian-integrated nutrition care.
Our in-house dietitian, Charné, joins the practice on Fridays, so nutrition therapy is part of your treatment plan rather than a separate referral.
Musculoskeletal physiotherapy.
Our physiotherapist, Tharusha, is part of the practice’s holistic model, supporting mobility and physical wellbeing as your weight and activity levels change.
On-site blood tests at LANCET Laboratories.
Blood and pathology testing is conveniently available on-site through LANCET Laboratories, located adjacent to the clinic. No separate trip across town for routine bloodwork.

Your Weight Loss journey: What to Expect
Step 1: Getting to Know You: Pre-Consultation Workup
Before your first consultation, you’ll be asked to complete a detailed food, diet, and lifestyle questionnaire. This helps us understand your day-to-day habits, identify possible dietary or lifestyle triggers that may be contributing to weight gain and review your medical and surgical history in advance.
Completing this beforehand means less time is spent on paperwork during your appointment, and more time is available to focus on your program and a thorough evaluation.
Step 2: The Initial Consultation (60 minutes)
Your first consultation is a full one-hour session with Dr Petropoulos. This includes: An explanation of the core physiological principles behind weight gain and weight loss, based on current medical evidence. A discussion of which weight loss program best suits your needs and preferences, either the Equilibrium Diet (a plant-based approach) or the Reset Diet (an anti-inflammatory approach). Baseline physical measurements, including weight and body circumference measurements at multiple sites. A full body composition analysis using the InBody 270 bio-impedance machine, with results explained in detail during your visit. Blood tests are requested where clinically appropriate, to help stratify your individual risk profile and identify any underlying medical causes that may need to be addressed for effective, sustainable weight management.
Step 3: A Personalised Medication Strategy
Where medication may assist your weight loss journey, treatment options are discussed based on your individual phenotype: the particular pattern of factors driving your weight gain. Matching treatment to phenotype is an active area of obesity medicine research; studies such as the Acosta phenotype study have demonstrated that identifying a patient’s specific phenotype and selecting medication accordingly can meaningfully improve treatment response compared to a one-size-fits-all approach. Medication options that may be discussed include weekly injectable therapies such as semaglutide or tirzepatide, as well as oral options such as Contrave, among others. Selected according to what is most appropriate for you. Your current medications and supplements are also reviewed as part of this process. Many common medications can contribute to weight gain without patients realising it, and where appropriate, alternative options may be suggested.
Step 4: Ongoing Monitoring & Follow-Up Care
Follow-up visits are recommended every 4 weeks. At each visit, we reassess your weight, body circumference measurements, and InBody body composition results to track your progress objectively over time. Each follow-up also includes a short educational session on a specific dietary or lifestyle strategy relevant to weight loss or the prevention of weight regain. Dr Petropoulos’s approach focuses on addressing the core hormonal drivers of weight gain, with the aim of correcting the metabolic patterns established during periods of weight gain or obesity and restoring the body toward a leaner, metabolically healthy baseline.
Conditions we treat: obesity, metabolic syndrome and prediabetes
Equilibrium Health treats a specific set of interconnected metabolic conditions in depth. Each has its own dedicated page if you’d like to read further:
Obesity Medicine
What obesity medicine actually involves, the bio-psychosocial model behind our approach, and the clinical foundations, namely pathophysiology, neurology, endocrinology, gut health, and immunology. ALL shape your treatment.
Metabolic Syndrome
How central obesity amplifies hypertension, insulin resistance, abnormal cholesterol and fatty liver. Find out why treating the shared root cause often improves all four at once.
Prediabetes
The evidence behind reversing prediabetes through weight loss and structured lifestyle change, with or without medication, and why early intervention matters.
Publications & Evidence-Based Medicine
The published research behind how Equilibrium Health approaches treatment and why the evidence points toward the maintenance phase, not just the initial weight-loss phase, as the real measure of success.
